Welcome to Willow Tree! Before becoming a new patient you must:

1. Be a Nebraska resident.

2. Fill out this New Patient Medical Form. It is a downloadable pdf which you can print and fill out. Then return it via fax (402) 620-4322; or scanned password protected pdf to: WillowTreePsychiatry@gmail.com

3. Finally, after review of your New Patient form, you must be accepted by our practice. Acceptance of new patients are based upon several factors, including: A) Availablity. There are times when we cannot accept new patients simply because we do not have openings. B) If we can help you. Sometimes, after reviewing a patients records, it is determined that we are not the correct type of provider a patient needs. In this case we will try to refer you to the appropriate provider.

Finally we want to make it clear that we would NEVER discriminate against anyone for:
1. The medical condition they have; or
2. Their Race, Color, Religion, Sex, Marital Status, National Origin of Ancestry, Sexual identity or LGBTQ status.
